What Happens Next
Step one
Requests submitted through this form go to the scheduling team. Nothing about your eyes or your health is needed at this stage, only how to reach you and when.
Step two
Someone will contact you within the time window you selected, to talk through what you have been experiencing and work out which office is most convenient for you.
Step three
Where relevant, the team will check your insurance benefits and explain what is covered and what is not before anything is scheduled, then book your consultation.
Step four
Allow longer than a routine eye appointment. Your eyes will be dilated, so arrange a ride and bring sunglasses, along with your glasses, medication list, and any previous surgical records.
Before You Write
Sudden loss of vision in one or both eyes
A new shower of floaters, flashing lights, or a curtain across your vision
Significant eye pain, particularly with nausea or headache
An eye injury or a chemical splash
Sudden double vision
These need same-day assessment. Contact an eye doctor's office directly or go to your nearest emergency department. This form is not monitored around the clock and is not an appropriate route for an emergency.
Your current glasses or contact lens prescription
A list of your medications, including eye drops
Your insurance card
Records from any previous eye surgery, particularly LASIK, PRK, or RK
The name of your optometrist or primary care doctor, if you would like findings shared
